Creating a form in word

I need to create a form in Word 2002. I have put in the "Text" fill-in fields, but I need to have a box where the fill-in field will be a picture that the user can choose from any picture on his/her computer. When I lock the document, it allows me to fill in the text boxes, but will not let me put a picture in the picture box (frame). How can I do this?

That won't handle pictures.
