Creating a 'smart' form

I'm trying to make a form with "toggle fields".



I need something like "Room to be cleaned ?" and by right clicking with
mouse to get the square check marked.



Also a text like "The house has 1 room" - right clicking 1. choose/select
between 1/2/3/4/5



I played arround with autotext but couldn't get i working :-(
