Error Message "Array Index out of bounds"

Recently I installed Office 2003 Prof. and have had loads of trouble. First my modem didn't work, then my D: drive wasn't being recognised (I had it replaced). The Photoshop wasn't working and I had to reload all my preferences. Now I can't enter anymore Business Contacts. I have received these 3 error messages whilst in Business Contacts when trying to enter a new business contact.

I have Windows XP system.

1. The form required to view this message can't be displayed - contact your administrator"

and 2. "Array index out of bounds"

also 3. "Run-time error '621'. Input past end of file"

Please help. I have tried looking up the error messages to no avail.

BCM sometimes has what's known as a "forms cache" problem. Clearing the
cache usually fixes it. Choose Tools | Options | Other | Advanced Options |
Custom Forms, then click Manage Forms, then Clear Cache.

If that does not work (I'm sure it will) but you can wipe it out be deleting
frmcache.dat
