Help Creating Custom Order Form Template

i am looking to create a custom template for my embroidery business.
essentially, i offer customers options in font, picture and name and i would
like to have an order form that lays out their customization options

for example, there would be four fonts that i would like to display for the
top of the design

there are 2 choices of picture

there are three choices for bottom of the design, each available in the 4
fonts described above

i'd also need to collect all personal information from the client, as well
as payment info

any templates exist in which i can switch out some photos/fonts?
do people create templates based on customer needs?

any help is appreciated

I don't think you're going to find anything this specific.

I don't see a whole lot in the roder forms

but you might be able to modify some of the product catalogs -- or maybe
something there will at least give you ideas.
