Help with designing a task as a form

Apologies if this has been asked before. This is my first time on here.

I'm trying to design a task as a form so it can be used over and again.

I always want it to generate a new "log" number every time a new form is
created.

I've got some way to doing it, but I don't know the correct formula for the
new number every time.

I have a screendump of what I've done so far, but it seems I can't add it in
here. If anyone can help, please email me and I will send you the screendump.
