What you want to do *can* be done but not easily. It would be easier to
start from scratch and make a form.

Output it as a .PDF, then make it a fillable pdf.

You'd need Acrobat to do that.

What verison of Word?

In 2003, and maybe earlier versions, Office comes with Microsoft document
scanning and imaging. That is specfically designed to scan and set the
document up for manipulation by other programs. Usually these programs are
found under the Microsoft Office menu item in the Start Menu.

Just start MS Document scanning with your form in the scanner. Then when
the document is scanned tell the MS Document scanning to send the document
to Word. You should now have a fillable form as a Word document. You may
have to do some editing work to get it to look exactly (as much as is
possible) like your paper form. I'd never done this before but I tried it
just now and it worked beautifully. with a form I had handy.
