How do I creat a form that allows for dataentry?

I would like to know a way to create a document (Word or Excel) that would
allow me to send it to people to fill out and return it back. Basically like
the forms I see on the WEB requesting things like name, address, a few
comments, and the like. Can it be done using Office or am I limited to only
create it via the WEB?
