You'll first want to insert the image into your document as if you
were creating a watermark. Then you can use some tables to help you
set up the format and insert form fields from the Forms toolbar into
the locations where you would type. It can be tricky getting it just
right. You will probably have to fiddle around with font sizes and do
a few formatting tricks to get it setup right if the form is complex.
The table format will help you keep control over the form.
Then you'll also want to click Tools > Options > Print and choose to
only print the form data so the image doesn't print, too.
Another way is to insert a frame, drop the image in the frame and then
lay another frame over the image and put the form fields in the
second/top frame.
