How do I set Word from that outputs as raw data?

I am creating a Word form using text form fields and drop-down menu form
fields. When the user has completed the form, Word must generate the raw data
into a new file that user can update.

There are two restrictions on the process:
1. The original form data cannot be updated by the modification process.
2. Similarly, the modified data in the new file must retain its changes.

I have designed the form document, and the template for the new report. But
I have not been successful in ensuring that the modified data is retained.

Can anyone recommend how I can fix this problem?
Can anyone help me out?

What do you mean by "raw data"?

How are you presently getting the data from the form into the report created
from the new template?
