How do you get a hyperlink to repeat in all records?

I have a meeting request form that needs to be completed by individual and
approved by supervisor via e-mail submission. I have a mailbox in Outlook to
direct the individuals request form which is then forwarded (by me) to
supervisor for approval. I'm trying to set up this form so the individual
simply hits a submit key and it goes to the Outlook mailbox. How do I get
the hyperlink to automatically repeat for each record? How do I get the
information on the form in to the e-mail message? Any help would be greatly
appreciated. Office 2003 XP.
