importing Word footer and TOC

one time I imported a Word doc into Publisher and the footer (which also had
an image in it) and an automated TOC converted over into Publisher like silk.
I tried it again and now the image (from the footer) appears at the top of
the document and the TOC didn't hook in. Why did it work the first time and
now it won't?
