mail merge custom form fields from outlook into word

I have created a custom outlook contact form. I want to make the custom
fields function for a mail merge of those fields into MS word documents.

Make sure the fields are defined in the folder (they'll be visible in a folder view) and that you start the merge from Outlook, not word.
