Outlook is changing the "From" address on its own

When I send an email to a specific address, Outlook will ALWAYS change the
account that it uses to send the mail. It only does this with specific
addresses.

Also, when I go to send the message, it will add to the number of processes
that it goes through.

I don't understand what's going on here, and this behavior is making Outlook
so unreliable.
