Outlook Public Folder Helpdesk Form

Does anyone happen to have an IT Helpdesk form for Outlook that I can use in
my public folders?

Or does anyone know where I might find one or some?

I know I can create one of my own, but it's just one more thing that's going
to vacuum up time that I really don't have time to generate...
