Picture Position Options in Word 2007

I'm creating a template in which I want to store graphical icons in the
Building Blocks to be inserted throughout the document.

My document has mirror margins, a gutter, and a wide "outer" margin for
these icons and also for text box call-outs. In the Advanced Layout dialog
box, in the tab labeled Picture Position, there are options for "Book Layout"
which include supposed placement of the image or text box "inside" or
"outside" of the margin. None of these options seem to do what you would
expect when selected.

In fact, none of the options in this entire dialog box really does what
you'd expect. I have consulted reference books and have trolled the web for
literally hours looking for documentation on these positioning options, but
there seems to be NOTHING that explains what they're supposed to do.

Does anyone have any resources on the options this dialog box?
