Pop up window

I want to have an on-line brief, at the conclusion, you would have to select
next. At that time a window would pop up and prompt you for name. After
typing and hit ok, a certificate would appear with a name. Can this be done?
If so, could someone assist?

For the pop-up window, try Spawn from
http://www.jimcosoftware.com/addins.aspx

The certificate will be a confirmation page brought up from the form the
name is typed into.
