Printing & Downloading Forms

How can I print a form that has been created in Outlook
2000? Obviously, File and then Print; however, when I try
this method, only the infomation in the message box prints
out and not any of the fields that I have created in the
form.

Further, how can I download the information in the custom
fields into excel so that I can review, consolidate and
manipulate the data?

Any assistance you can provide would be greatly
appreciated. Thanks!
