Sample Form

I need to create a simple form in Access to conduct a
survey. Any suggestions where I can find a sample form
which would give me some directions.

There are many templates included with the Access program. Start there.
Secondly, first decide on the design of your table to hold the survey data.
The fields in the table will hold the results of your survey. Then use the
form wizard to create the form based on the table. Access is a relational
database program, so if you only want a flat file, maybe excell would be
adequate for your purposes.
