setting up emails for passing Tel messages

Can anyone help me with the creation of an automated email
message for recording and sending the information that is
received via the telephone for someone.

Have tried to create a base template and insert the users
name, but could not findout how to set the date and time
of the call as auto date & time.

Have tried using word to create the template with the auto
information, which worked. Need help setting the page up
so that it automatically opens the email dialog options
and setting the email address up automatically.
