tables will not size

I have worked 2 days on this. I have a table that the cells do not align.
I have tried fit to contents, window, fixed column width and just about every
other thing that i could thing of. I have tried to align them all manually
to no avail. Also a lot of the table is off of the screen. Hope someone can
help.
