I need small folded cards to use partly as place settings but with a border
and the following text

Millbank & Tillyfourie Bowling Club

2 Bowl Pairs
Winner 2007

Phillis Forbes

How do I set up a templete so that I can get this size? (Text given to
indicate my needs and not for you to spoonfeed me!)
