Unwanted format changes applied to entire document.

When I highlight a word, sentence or paragraph and apply a format change
(bold, underline, number, indent, etc) the format is applied to the entire
document. When I click on the UNDO button the formatting is removed from
everything except the origninally highlighted selection. I noticed also that
every format change automatically adds a new format to the Styles list.

I can work on a document during several different sessions without this
happening. But once it starts it happens on every document I open (even new
documents) until SOMEHOW it stops. Very frustrating and time consuming!

How/why does this start happening and how do I stop it?
