Word userform examples to dl?

Are there any example USERFORM word documents out there that we can dl? I'd
like to take a look at at least 2 or 3 to make better sense of the
instructions I've been reading over last couple of weeks.

You can grab

This contains a sample template with boilerplate text and hidden text within
bookmarks. An autoexec macro shows a userform with checkboxes; checking a
box unhides the contents of the corresponding bookmark.
