allowing a user to attach a file to a form

Word 2000. I have a requirements form that needs to be filled out by a user.
In some cases, the user has a MS-WORD or MS-EXCEL file which includes much
more detail of the need. Is there a way in a form field (text or other) to
allow attachments to be added.

Use the Insert > Object command. In the dialog, click the Create from File
tab, check the option for "Display as icon", and enter or browse to the file
to be included.

To read the attached file, double-click the icon just as you would in
Explorer.

Thanks Jay, but as soon as my form is "locked" and someone is entering
information, my Insert menu has all the submenu items grayed out (except for
Picture).

Thoughts?

Is there a Form toolkit item which allows a user to attach a file into a form
field in a locked form?
