are there any sample templates on how to ask for a raise

looking for a sample letter to send to my boss to ask for a raise. he has
requested that i put my request in writing...any suggestions

Since your boss asked you to submit your raise request in writing,
I think you need to compose the letter in your own words and not
rely on some sort of template. The letter should sound like you wrote
it and should include all your accomplishments and contributions
relative to the profitable success of the company you work for.
