charts in word are losing data & formatting after saving

When I re-open my word documents that have excel charts in them, the newest
data added to the chart table and the formatting are lost once i open the
chart to edit it again. What is going on?

Version of Word? How did you create this chart object? Did you copy/paste from
another one in the document? Create it from scratch? Import it from Excel? If
the latter, is there still a link to the original?
