Developing a Form in Word and Locking the Format

Is there a way to distribute a Word document that only
allows entries in select areas or fields of the dicument
and does not allow changes to the instructions, field
headings, etc.?

We want to distribute a Word document to be used to report
information but do not want to allow the user to change or
delete the form in any way.
