Does anyone have a Eye Examination template for Word?

I need a template for eye examination for my Family Practice. Does anyone
have one to share? Thank-you for your consideration.

Since the size of the letters is what determines the patient's vision, you
probably can't make one yourself. My suggestion: Spend the money and buy a
Snellen chart. They only cost about $15-$18. (My eye doctor hasn't used a
paper/card chart for years and years and years. It's a projected image from
some sort of a computer.)

JoAnn, First, Thank-you for your reply. But, I do not need a Snellen chart,
I need a template for the eye examination. That would be a form, maybe used
by other providers to document the results of the examination. We need one
in our office and I was hoping I could find one that someone else is using,
if they would share it with me. Thank-you for your assistance anyway.

Ohhhhhhhhhhhh - that's a different critter.

I know there are standard forms for other medical examinations so I would
think there's something available commercially. I'd suggest buying 1 pack
the create your own template based on that, modifying to suit your specific
needs.
