Forms with a picture

I created a form with various types of text boxes/drop down boxes.
The form is locked for users to fill in the necessary information.
The problem is that I would like them to be able to put a picture in
(different photo most every time). I cannot find a box that will
allow a photo, only text. I tried to put in a continuous break before
& after where the photo is but it doesn't work as I have text boxes to
the left of the photo ie:

Name: (text box)

Address: (text box)
Medium sized
photo here on the right
Height: (text box)

Weight: (text box)

Is there any relatively easy way to do this? Any assistance would be
appreciated.

Relatively easy? No... but there is a way. Download the form_picture
demo from http://jay-freedman.info and examine the way it works. There
are multiple moving parts, including MacroButton fields, bookmarks,
and macros. Getting the names of things correctly set and getting the
bookmarks correctly placed are critical steps. If you have trouble
implementing the scheme in your own template, write to me.
