Help with a code

Hi,
I would like to create a table (or cell) with a note text and a "don't show
this note again" check box so that when the box is checked, the note will
not show again. How do I go by?

Requires passing a "cookie" to the visitors computer.
Very unreliable since a lot of users delete cookies on a regular basis.
