How do I create a form

I want to create a form to enter data into a table.

here is how I would like it to work.

The user would select one value in list for field #1, then in field #2 a
list would appear that would be dependent on the value selected in field #1.

For example

If I were to select:

Ice Cream from table 1 in field one - I would be given a new set of choices
like (strawberry, chocolate, vanilla) from table 2 to enter into the field
number 2.

But if I were to select Cheese from table one - then my set of choices in
field 2 would be (parmesan, cheddar, mozza).

Or if I were to select Beverages from Table one - then my set of choices in
field 2 would be (milk, yogurt drink, etc.)

The tables would be longer than 3 choices, but this shows the relationships.
