How do I set up a numbered list of fields in a template?

I am setting up a template for work, and I need to set up numbered fields
where people can input the information,

ex.
1. <Service> - <Description of Service>
2. <Service> - <Description of Service>
3., 4., etc.

My issue is that sometimes there might be only 2 listed items, while other
times there might be 6+, so my question is whether I can put in just 2 or 3
of those and then if they need more, the fields just keep going down the
page, similar to when you create a regular numbered list and the numbers
continue down the page everytime you type in something and hit the return key.

I would create a userform with a list box and a text box. Enter each item
into the text box, then click on a command button to add it to the list.
When finished, iterate through the items in the list, entering each one into
the document with a sequence number and a tab before it.
