It depends on what sort of picture it is, but the answer is 'probably not'.
For a bitmap you can extract the image
http://www.gmayor.com/extract_images_from_word.htm and use a bitmap editor
to make the changes.

If the graphic is a bitmap you can open it in
another graphic editor (MS Windows Paint or
http://www.irfanview.com for example) and add text
to it, or in Word you can insert a text box in your
document, add text to it, format the background to
be 'no fill' and then use the Drawing Toolbar
(Draw=>Group) to combine it with the picture.
