MS JET OLE DB Initialization Information

I amtrying to modify an existing InfoPath form that directly connects to an
Access database. When I try to preview the form I receive the following
dialog box

Please Enter MS JET OLE DB Initialization Information

Data Source: \\...\formdata\xxx.mdb
User Name: Admin
Password:
Provider String:
Open Mode: DB-MODE-READWRITE

Button “OK†Button “Cancelâ€

If I enter the correct path for the database, it just pops back up.
