Outlook attachment embeds in body of email not attachment 'bar'

When I send an email with an attachment it won't insert in the "attachment
bar." Instead it is embeded in the "body" of the email. What?

Try changing the message format to plain text or html. (note: using word as
the email editor will also influence placement.)
