Your server must be running Front Page server extensions and have SMTP
services for outgoing mail:

1. Create a new form in FrontPage by opening a new file and inserting form
controls (drop downs, text fields, etc.)
2. When finished, right click the form and choose to e-mail the form. Set
the e-mail address
3. Page through the other tabs and set other options you wish
4. Its always good practice and more friendly to create a confirmation page
to let users know that their form submitted successfully and what action you
will take (get back to them soon, send out requested items, whatever). To do
this, make another page with your "thank you" message. If you want to get
fancy, you can include some of the data they submitted to you in your
confirmation page, such as thanking them by name. To do this, choose
Insert, Web Components, Advanced Controls, Confirmation Field.
