sendobject macro

I am trying to create a macro that e-mails information from a form to a
specified recipient included on the form. The macro currently attaches the
form to the e-mail but I can not figure out how to populate the "To" line
with the recipient listed on the form.

Thanks for your help, I'm now one step closer...

How about attaching two fields from the same form in the Subject Line?
=[Forms]![NameOfForm]![Name] and =[Forms]![NameOfForm]![Id number]

I have tried the above syntax 20 different ways...
