Strict formatting of an Index in Word

I'm writing a book that requires me to use specific fonts for
everything. I'm trying to get Word to build my index automatically,
but I can't get control of the fonts it uses. This is really messing
things up. Here is what I've tried so far:

1. Use the built-in templates: when I click on the template name, the
Modify (format) button becomes disabled. I want to override the fonts
used in a template but it won't let me.
2. Use the From Template option: I click on this option and the Modify
button works! If fix my fonts and build the index. Voila! The fonts
are perfect. But wait a minute - there isn't an Index Header ("A",
"B", etc). All the other pre-formatted templates have index headers,
but the Use Template option doesn't? I built them all manually, but
now I need to update my index and they get lost again. This isn't a
long-term solution.
3. I went back to the built-in templates (with index headers) and
after building the Index I right-click on an entry and select the Font
option. I change the font to be correct and close the dialog box.
Well, the font is updated for that entry, but ONLY that entry. I would
have to manually do this for every entry in the index.

Isn't there a way to build an index using my fonts without having to
do something manually (and lose it all when I update the index)?
