System Maintenance Form - Outlook

I want to create a new Outlook form for use by my Technical Infrastructure
group to notify relevant users of system maintenance events.

I'm thinking that I want to create a form detailing site (with checkboxes
for sites effected), technical contact (checkboxes again), Start date/time,
End date/time, Description, Detail and post change checks (the last three of
these would be free format).

This sounds pretty simple, I know, but if possible I want the subject field
to populated with detail from the form.

Does anyone have anything like this already or can you point me in the right
direction. I made an attempt myself and it really was a bit of a mess...
