take off password on the 'submit' of an online form in frontpage

how can I take off a password on a form, when I don't even know how it got
there?
It appears after someone clicks on the submit button.
Thanks.

You need to have your hosting company check the permissions on the
_private folder
and make sure the server has write permissions set for it.
