why did my outlook form revert to an old version?

I designed a form and having been using it for months. Today, for no
apparent reason, it rolled back to an older version of the same form, losing
a pile of data! Why did this happen and can I get the form back or do I have
to recreate it?

How can we know with the few details you've given? Details like your Outlook version, where the form is published, recent changes you've made to your system, how you use the form, what kind of form it is -- may all be relevant.
