Outlook TASK template

I would like to get and/or create a custom TASK template for Outlook 2003.
This would be used for Phone Messages and/or walk-ins. I would like to like
to have selectable options for call back, urgent, etc... Also need 2 or
more phone numbers and message space. This would be used for tracking tasks
that aer not completed and have a history log of messages.

Do anyone know if there is a template like this or how to create one?
