Brian, in addition to Don's questions, have you also checked your envelope
settings in the Publisher Tools > Options menu? If you go to Tools > Options
Print Tab > and make sure you have the printer orientations setup
properly. Put a check mark in the box that says to use these settings, and
make sure orienation and placement are set.
