supply order request

I am trying to create a form for employees to request an order for a certain
supply. I want to see if there is anything that would make it look better
than just plain old words and lines. Does anyone know where I might find a
template for this? Or anything that might help. Thanks

probably a zillion ways to do this.

You can use Word and save it as a template. You can use Word and output to
Acrobat (if you have Acrobat - the writer not the reader) and then make it
into a fillable form.

I think Word even has a template for a purchase order you could probably use
that to get started and make it look pretty.
