Adding an attachment to a custom form in Outlook 98

I am using Outlook 98. I have a custom response form that
I wish to use and would like to fix an attachment to this
form.
I would like to have only the To, CC and a message field
available for input. I can do all but add the attachment .
Can anyone assist?
