im trying to set up a customer work log?

heating man needing some help

i have a small business and i'm trying to set up a basic work log to enter in
my customers and keep track of all work i've done for them i want to be able
to just enter in like there phone number and have there sheet pop up does
anyone know how to set this up and make it happen if so please help me with
it thank you...
